زمینه و هدف: تشخیص سرطان پستان، استرس زیادی را پیش روی بیماران و همسران آنها قرار میدهد. کاهش کیفیت زندگی و اختلال در الگوهای ارتباطی زناشویی از جمله مشکلاتی است که متعاقب ابتلا به سرطان پستان، توسط بیماران و همسران آنها تجربه میشود. مداخلات روانشناختی میتواند در کاهش مشکلات بیماری سرطان مؤثر باشد. این مطالعه با هدف تعیین اثربخشی برنامه آموزش مبتنی بر درمان شناختی- سیستمی گاتمن بر کیفیت زندگی و الگوهای ارتباطی زنان مبتلا به سرطان پستان و همسران آنها انجام شد.
روش کار: مطالعه از نوع نیمه تجربی با طرح پیش آزمون- پس آزمون و با گروه کنترل بود. افراد حائز شرایط، به شکل تصادفی در دو گروه آزمایش و کنترل قرار گرفتند. هر دو گروه، پرسشنامه های کیفیت زندگی بیماران مبتلا به سرطان پستان و کیفیت زندگی سازمان بهداشت جهانی و نیز پرسشنامه جدایی- فاصله گاتمن را تکمیل کردند. سپس، گروه آزمایش در جلسات آموزشی درمان شناختی- سیستمی گاتمن به مدت 8 جلسه دو ساعته شرکت کردند. پس از اتمام دوره آموزش و سه ماه بعد از آن از هر دو گروه، آزمون گرفته شد. داده ها با استفاده از آزمونهای تی مستقل و کوواریانس مورد تحلیل قرار گرفتند.
یافته ها: نتایج نشان داد میانگین نمرات الگوی ارتباطی (6/6±21/20) و کیفیت زندگی (11/79±67/22) در گروه آزمایش پس از مداخله، نسبت به میانگین الگوی ارتباطی (8/85±45/20) و کیفیت زندگی (12/08±98/50) در گروه کنترل، تفاوت معناداری داشت (0/001>p).
نتیجه گیری: براساس یافته ها، درمان شناختی- سیستمی گاتمن در بهبود کیفیت زندگی و الگوی ارتباطی بیماران مبتلا به سرطان و همسران آنها موثر است. بنابراین، این روش میتواند در کار بالینی به متخصصان حوزه سلامت و خانواده که در ارتباط با بیماران و خانوادهها هستند، کمک کند.
