زمینه و هدف: سیستم های درمانی علاوه بر توجه به کمک به بهبود سلامتی بیماران حین بستری لازم است به پیامدهای بعد از ترخیص آنها نیز توجه نمایند. لذا شناسایی عوامل مرتبط با پیامدهای بعد از ترخیص، حائز اهمیت میباشد. مطالعه حاضر با هدف تعیین ارتباط آمادگی بیماران برای ترخیص از دیدگاه پرستاران با پیامدهای بعد از ترخیص انجام شد.
روش کار: مطالعه حاضر از نوع توصیفی- تحلیلی بود که در سال 1401 انجام شد. جامعه پژوهش، بیماران بزرگسال بستری در بخشهای عمومی دو مرکز منتخب آموزشی- درمانی شهر سنندج و پرستاران هر کدام از بیماران در روز ترخیص بودند که از بین آنها تعداد 360 نفر با روش نمونه گیری در دسترس انتخاب شدند. ابزار جمع آوری اطلاعات شامل فرم اطلاعات دموگرافیک، پرسشنامه آمادگی ترخیص از دیدگاه پرستار و چک لیست 6 سوالی پیامدهای بعد از ترخیص بود. دادهها با نرم‌افزار SPSS-21 در سطح معناداری 0/05 و با آمار توصیفی و آزمون‌های آماری تی مستقل، کای اسکوئر، مدل رگرسیون چندگانه مورد تجزیه و تحلیل قرار گرفت.
یافته‌ها: میانگین نمره کل آمادگی ترخیص بیماران، 1/19±7/82و در سطح متوسط بود. کمترین نمره آمادگی مربوط به بعد دانش (1/60±7/61) بود. بهبودی بعد از یک ماه در 93/4 درصد بیماران گزارش شد. بیمارانی که در فاصله ترخیص تا یک ماه بعد فوت شده بودند به طور معناداری نمره آمادگی حین ترخیص کمتری از دیدگاه پرستاران، دریافت کردند (0/001<p).
نتیجه گیری: نتایج نشان داد که نمره کل آمادگی پس از ترخیص در حد متوسط بود. از سوی دیگر برای کاهش پیامدهای منفی بعد از ترخیص، ارزیابی دیدگاه پرستاری از وضعیت بیمار در زمان ترخیص و لحاظ آن در تصمیم گیری برای ترخیص باید مورد توجه قرار گیرد زیرا بیمارانی که فوت کرده بودند؛ به طور معناداری از دیدگاه پرستاران، آمادگی کمتری حین ترخیص داشتند.
